Immunization and Child Health Assessment
Seaman USD #345 School District abides by the State of Kansas Immunization (KS 72-5209) and Child Health Assessment (KS 72-5214) laws, and all students enrolling in this district shall provide written proof of current immunizations and for new students up to the age of nine years, shall submit evidence of a physical examination during the previous calendar year. For questions, please contact your building school nurse.

Athletic/Activity Paperwork
Before any student may try out or practice for any activity or sport, the school office must have the following on file.

• Assumption of Risk Form
• Pre-Participation Physical Form
• KSHSAA Concussion Guidelines & Release Form

Breakfast is an important start to the day!
Students perform better with morning food intake! They work faster, make fewer errors, have more energy and greater physical endurance. All Seaman schools have breakfast programs.
